With a confirmed case of COVID-19 in our community, we will be rescheduling all non-urgent visits until further notice. We will be offering phone visits only for matters that can be reviewed by phone. If you require an urgent appointment, please call to speak with a staff member. At that time, you will be screened for COVID-19 exposure. We will give you directions on what to do next if you screen positive. Thank you for your cooperation.

Expected to open late 2019, this location (200 Larch Street) will focus on the marginalized and homeless population in partnership with the CMHA of Sudbury/Manitoulin. This funding will help to ensure quality primary health care is more accessible to people in the Sudbury District living with medical conditions, mental illness, addictions and poverty closer to home.

NP Outreach - Making a difference

community mosChapleau

The NPs at SDNPC have provided outreach well woman clinics to residents of the underserviced town of Chapleau since 2007. On a rotating schedule, NPs would travel to Chapleau for one week every 2 months to provide these services. With the development of a Family Health Team in Chapleau, residents are now able to see local health care providers thus our services are no longer required at this time.

Lively High School

For a period of 1 year, SDNPC provided temporary onsite access to primary health care services for the students of Lively High School.
