Workshops on Diabetic Overview and Complications of Diabetes will be held on request

Purpose:

 To provide family focused diabetic education that promotes self management of the disease.  The focus will be on achieving recommended targets for blood sugar, HbgA1C, and cholesterol in recognition that each target is of equal importance.

·         Pathophysiology and complications

Smoking Cessation E-mail to a friend

Providers at Sudbury and District Nurse Practitioner Clinics support patients who want to stop smoking. Smoking status is noted on all patient charts. Smoking patients are assessed at interval to determine their readiness for change. Our providers offer a wide array of supports to smokers who are considering quitting. Approaches often include teaching, supportive counseling, nicotine replacement therapy and medications. We also work closely with the Canadian Cancer Society’s Smokers Helpline Program. Smokers who are getting ready to quit are offered referral to this program that provides telephone teaching and support through all phases of the quitting process.

HPV immunization Program E-mail to a friend

A series of 3 immunizations provided by a Nurse Practitioner to females between the ages of 9 yrs through 26 yrs.  The vaccines may be purchased via the clinic for individuals who do not have insurance coverage. 

The HPV vaccine helps to protect against infection from HPV types 6, 11, 16 and 18.  These viruses are associated with Cervical Cancer, Abnormal Cervical cells and Genital Warts.

The HPV vaccine does not protect against other sexually transmitted diseases not caused by HPV.

Why Weight? E-mail to a friend

The Why Weight Program was developed to address the health care crisis known as obesity and overweight. Several studies have shown that patients who carry excess weight are at a significantly higher risk for the development of chronic disease (ie. Diabetes, cardiovascular disease, etc). The program is voluntary and available for all patients who are identified as being overweight or obese. Patients are provided with a package of information containing helpful hints and tips about how to lose weight (ie. Behavior modification, reading labels, shopping & dining out, fitness & exercise). The package also contains a food diary and fitness log for patients to use to track their progress.

Patients are also offered monthly weigh-ins, and for those who need extra encouragement, weekly weigh-ins may be offered as well. There is no cost to the patient.

 
 

Health Tips

Limit Sun Exposure
Extended exposure to the sun can increase your risk of skin cancer. If you are going to be out of doors for extended periods, wear clothing that will protect your skin and be sure to use sun block on any exposed areas.
